Who we are
Our Community Integrated Advance Care Planning Service delivers education to health and social care professionals in advance care planning and end of life care tools.
The team is led by Dr Seamus Coyle, the Community Palliative Care Consultant for St Helens and Knowsley, and work in partnership with Willowbrook Hospice.
We aim to deliver consistent and high quality education on the implementation and sustained use of all nationally recognised end of life tools such as, the Gold Standards Framework and Preferred Priorities for Care. Using regional education programmes such as the Six Steps to Success, Opening the Spiritual Gate, Advance Care Planning and more locally the Care and Communication Record and the Citizens Charter for End of Life Care.
Who is our service for?
Our service is for Knowsley and St Helens health and social care professionals working in the NHS, private, voluntary sector and charitable organisations.
The team work closely with the Clinical Reference Groups for both St Helens and Knowsley and take regional guidance from the North West Coast Strategic Clinical Networks. The team provide full and half day education sessions with the primary aim to educate as many staff as possible in the principles of Advance Care Planning in order for the skills be implemented in practice.
Where else do we provide this service?
Education sessions are delivered centrally in Willowbrook's The Living Well and the team will deliver bespoke education sessions within each of the localities of St Helens and Knowsley. We engage with the general public through attending voluntary agencies and Carers Center meetings via invitation, and we host an annual Dying Matters event to raise awareness of the importance of making future plans for your care.
How to access our service
The ACP team have a quarterly newsletter that is distributed to community teams, care homes, CCG members, specialist teams, end of life champions, domiciliary, and local council quality teams. The newsletter has education dates and good news stories / star recognition awards. You can book via the telephone number below or via email:
