Who we are
Our dedicated district nursing service provides high quality nursing care and treatment to patients in their own homes (including residential homes) 24 hours a day, 365 days a year.
We treat patients who are housebound or require treatments where a home visit is necessary. The service offers professional advice, support, teaching and skilled nursing care to enable people to live as independently as possible with an acute or chronic illness/disability who have a nursing need.
We also run treatment rooms for patients who are able to leave their home and for whom it is clinically appropriate to do so.
Who is our service for?
The service supports patients aged 16 and over who are registered with a Knowsley GP. Examples of the care we provide includes:
Holistic assessment to look at all areas of your physical and mental wellbeing
End of life care
Administration of injections / medications
Who do we work in partnership with?
We work closely with patients, carers, GPs and other health and social care professionals to help you to live as independently as possible.
How to access our service
You can be referred to the service by your GP or other health and social care professional. We also accept self-referral.
Following a referral, your community nurse will undertake an assessment to discuss your needs and agree a care plan with you.
