​Who we are
Our Healthy Knowsley Service can support you to make small changes to your lifestyle that result in a big difference to your health and wellbeing. The service will support you to set achievable lifestyle goals, allowing you to meet long term aims.

Who is our service for?
Our service is open to adults (18+) and families in Knowsley who want to take steps towards a healthier lifestyle. You may want to focus one or more of the following areas:
  • Healthy eating
  • Becoming more active
  • Weight management
  • Stopping smoking
  • Sensible drinking
  • Preventing diabetes
Who do we work in partnership with?
We work in partnership with a number of services to ensure that you get the right support to meet your lifestyle goals. These include:
  • Changes Weight Management
  • Family Futures (family weight management)
  • Slimming World
  • Volair Activity for Life Clinical Programme
  • Volair Community Wellbeing Programme
  • Smokefree Knowsley
  • Knowsley CGL (drug and alcohol services)
How to access our service
You can self-refer to the Healthy Knowsley Service by phoning our free phone number 0800 0731 202 or you can talk to your GP or health care professional who can make a referral on your behalf.

Once we receive your referral, one of our wellbeing advisors from our Lifestyle Hub, will contact you by telephone to discuss the options available help you to achieve your goals. We will listen to how you want to make the changes and give you information on the services that can best support your needs. You will choose the support that you need and we will then make an onward referral to a specialist service or to see one of our wellbeing coaches.     

What will happen at your first appointment?
Your first appointment will be a 45-minute appointment with a wellbeing coach. These appointments are available in Health Centres across the Knowsley area and are available during the day and of an evening. You will choose a venue and time that meets your needs when you discuss the referral with the wellbeing advisor.

The wellbeing coach will discuss your reasons for wanting to make changes and support you to develop a personalised health plan. This health plan will be led by you and will be made up of changes that are realistic and achievable for you, that fit in with your lifestyle and can have a big impact over a period of time.

How many appointments can I have with the wellbeing coach?
You can have up to six appointments with your wellbeing coach. These appointments will be spread over a period of time that suits you. Some people like appointments in quick succession whereas some people prefer longer term support. At each appointment, you will discuss the progress you have made in achieving your goals, and set new goals that help you to progress towards long-term sustainable lifestyle changes.
