School Health Team - Sefton


​Who we are
We are part of the integrated 0-19 team working in conjunction with the Health VisitingTeam

Each school has a named School Nurse who work together with the wider team to deliver the Healthy Child
Programme to children aged 4-19 years within the borough of Sefton.
The team consists of School Nurses (some of whom hold a Specialist Practitioner qualification), Community Staff Nurses, Clinical and Administrative Support Workers

As a team we offer:
  • Advice, information and support on all aspects of health and wellbeing encompassing both physical and emotional health.
  • A health needs assessment in the form of the Lancaster model online questionnaire which is completed by parents of reception aged children and pupils in years 6, 9 and 11. (Letters will be sent to parents during the school year to inform them of when these will be completed)
  • Health screening for all reception aged children which includes height, weight, vision and hearing.
  • The National Childhood Measurement Programme (height and weight measurement) is offered to all pupils in Reception and Year 6.
  • Annual health assessments for looked after children.
  • Support for young carers.
  • We play an active role in safeguarding vulnerable children and young people.
  • We work with families, children and young people and can offer health assessment, targeted interventions and further referrals according to needs identified.
Which team covers where?
Sefton has seven geographical 0-19 teams based on six office sites. Below are details of which team covers, which school. Each School Health Team has responsibility for all school age children who attend school in the geographical area of Sefton referred to below.   • Bootle 0-19 covers All Saints, Bedford, Christchurch, Hillside High, Hugh Baird Studio School and Sixth Form, Lander Road, Linacre, Our Lady Star of Sea, St. Elizabeth's, St. Monica's, St. Phillips (Bootle) and Thomas Gray.

  • Feelgood 0-19 covers English Martyrs, Grange, Hatton Hill, Hawthorne's High, Holy Spirit, Litherland High, Litherland Moss, Netherton Moss, Our Lady Queen of Peace, Our Lady of Walsingham, Rowan High/Park, Savio High, Springwell Park, St. Benedict's, St. Oswalds and St. Robert Bellarmine.

  • Formby 0-19 covers Ainsdale St. John's, Clarence High, Freshfield, Formby High, Kingsmeadow, Merefield, Our lady of Compassion, Range High, Redgate, Shoreside, St. Jeromes , St. Lukes, Trinity St. Peters and Woodlands.

  • Hampton Road 0-19 covers Birkdale High, Birkdale Primary, Christ The King, Farnborough, Greenbank High, Kew Woods, Linaker, Our Lady of Lourdes, St. Phillips (Southport) and St. Teresa's infants Infants.

  • Hoghton Street 0-19 covers Churchtown, Bishop David Shepherd, Holy Family, Holy Trinity, Larkfield, Marshside Primary, Meols Cop High, Norwood, Peter House, Prestfield, Stanley High, St John's C of E Primary and St. Patricks.

  • Maghull 0-19 covers Aintree Davenhill, Deyes High, Greenpark, Holy Rosary, Hudson, Lydiate Primary, Maghull High, Maricourt High, Melling, Northway, St. Andrews, St. Georges, St. Gregory's, St. John Boscoe, St. Thomas and Summerhill.

  • Prince Street 0-19 covers Chesterfield High, Crosby High, Forefield Infant School, Forefield Junior School, Great Crosby, Holy Family High, Merchants Boys High, Merchants Girls High, Merchant's Prep, Newfield, Olsen House, Rimrose Hope, Sacred Heart High, Stanfield,  St. Edmonds & Thomas, St. Johns Waterloo, St. Luke's Halsall, St. Mary's High, St. Michaels High, St. Nicholas, St. William of York, The Mount, Ursuline, Valewood and Waterloo Primary. 
Who is our service for
We deliver support, advice and interventions to children, young people, families and schools within Sefton.

We strive to promote and protect health and to prevent ill health by delivering a high quality service, which is responsive to local need. This is achieved through working collaboratively with our partner agencies including Schools, Children’s Centres, Statutory and Voluntary agencies.
 
In addition, we work closely with schools to develop health policies and promote good health and attendance, by supporting them in delivering information to pupils on a wide range of health related subjects, both within lessons and at health events.
 
If your child has a medical need, we are here to support you and the school in completing a health care plan. We will also offer assessment and ongoing support through early help and intervention services.

If you have any health concerns for your child please contact your school nurse to discuss these further. We will offer advice and support and can make further referrals if required.

Who do we work in partnership with
Schools, Children’s Services, Specialist Nurses, Enhanced Services Team, GP's, Children’s Centres, Active Sefton.

We also work alongside the Immunisation Team, delivering immunisations to children and young people in accordance with the UK schedule.
