Sustainability and transformation partnerships

The NHS Five Year Forward View was published by NHS England in October 2014 identifying the increasing gap resulting from growing demand for NHS services. The strategy highlights several areas for change to address this gap and make sure NHS services can continue to be safe, effective and sustainable after 2020/21.
The NHS Five Year Forward View noted a gap between where we are now and where we want to be in five years' time, with three areas highlighted:
  • The health and wellbeing of the population
  • The quality of care that is provided
  • NHS finance and efficiency of services
The NHS and local councils have formed partnerships in 44 areas covering all of England to improve health and care. Each area has developed proposals built around the needs of the whole population in the area, not just those of individual organisations.
The boroughs covered by our Trust sit within two of these areas – Greater Manchester (Wigan) and Cheshire and Merseyside (Halton, Knowsley, Sefton, St Helens and Warrington).
