Community Therapy Service - Knowsley


Who we are​
Our Community Therapy Service provides occupational therapy and physiotherapy support for people in Knowsley who may be having difficulty managing daily tasks at home due to illness or injury. Our aim is to support people to recover from illness and injury and promote independence so people can stay well within their own home.

Who is our service for?
Our service is available for people who are housebound due to illness or injury, who are aged over 18 and are registered with a Knowsley GP.

We support patients when they are experiencing difficulty managing daily tasks as a result of an illness or injury to help them recover previous functional ability and maintain their independence.

Who do we work in partnership with?
We work in partnership with other community-based services in Knowsley and can refer patients onto other services that will be able to meet their ongoing needs within the community, such as the Centre for Independent Living.

How to access our service
Your GP or any other healthcare professional involved in your care can refer you to our service. 

What will happen at your first appointment?
During your first appointment, a member of our team will come out to visit you at home to complete an assessment with you, which will help us to identify your needs and the goals you'd like to work towards together.

Following this assessment, we will work with you to support you to achieve the goals we identify during our first appointment over a period of six to eight sessions, where we will come out to visit you in your home.
