Who we are
Our Community Matrons provide complex assessment of patients with long term health conditions to proactively manage those high risk patients with complex long term conditions.

They aim to enable patients to self-manage their own health with support and to prevent unnecessary hospital admission and or facilitate earlier discharge from hospital.


Who is our service for?
Our service is for patients who are registered with a St Helens GP who require management of long term, complex health conditions.

How to access the service
Referrals can be made by any health and social care professional.
