Learning Disability Community Team - Halton

Who we are
Our service supports adults in Halton who have difficulties using mainstream adults' health services because of a learning disability. This could be for a number of reasons, such as problems communicating or problems with mobility. 

We will assess you to help us understand what is causing your issues and how we can help. Following this assessment, we will put together a care plan that meets your individual needs. This could include physiotherapy, occupational therapy, speech and language therapy, psychology services and psychiatry.

We can work with people in their own homes, at college, at our team base at the Bridges Learning Centre in Widnes, or at day services across Halton.

This may be on a one to one basis, in a group or by supporting family members and carers. We also provide training for family members, carers and other professionals supporting adults with learning disabilities.

Who is our service for?
Our service is for adults age 18 and over who live in Halton or are registered with a Halton GP who have a learning disability.

How to access our service
You can contact our team directly for an appointment or you can visit your GP, who will be able to make a referral to our service for you.

Once we receive your referral, we will then decide if we are the best people to offer support or if there's someone else who can help. If we can help, we'll contact you to arrange an appointment with our team.

Where else do we provide the service?
We also provide learning disability services for adults in KnowsleySt HelensWarrington and Wigan.
