Learning Disability Community Team - Warrington

Who we are
Our service supports adults in Warrington who have difficulties using mainstream adults' health services because of a learning disability. This could be for a number of reasons, such as problems communicating or problems with mobility. 

Our team is made up of specialist learning disability nurses, who are experts in working with people with learning disabilities, as well as a consultant psychiatrist, occupational therapist, speech and language therapists, physiotherapist and psychologists.

We will assess you to help us understand what is causing your issues and how we can help. Following this assessment, we will put together a care plan that meets your individual needs. This could include physiotherapy, occupational therapy, speech and language therapy, psychology services and psychiatry.

We can work with people in their own homes, at college, in outpatient clinics or at day services across Warrington. 

This may be on a one to one basis, in a group or by supporting family members and carers. We also provide training for family members, carers and other professionals supporting adults with learning disabilities.

Our Let's Check initiative
Health promotion is a big part of what we do and we are proud to have developed and implemented the Let's Check initiative across Warrington to improve the health and wellbeing of adults with learning disabilities.

They achieve this by supporting adults with a learning disability to access the health checks, appointments and treatment they need to enjoy positive health and wellbeing. 

As part of Let’s Check, all our service users have their own Health File – a bespoke, patient-owned folder that people with learning disabilities take with them every time they access health services. The file not only contains important personal information about the individual but also enables health professionals to check and record vital information to ensure continuity of care across the services they access.

Who is our service for?
Our service is for adults age 18 and over who live in Warrington or are registered with a Warrington GP who has a learning disability. 

We can also provide support, advice and training for health professionals working with adults with learning disabilities. To find out how we can help, contact our team directly.

How to access our service
You can contact our team directly for an appointment or you can visit your GP, who will be able to make a referral to our service for you.

Once we receive your referral, will then decide if we are the best people to offer support or if there's someone else who can help. If we can help, we'll contact you to arrange an appointment with our team.

Where else do we provide the service?
We also provide learning disability services for adults in HaltonKnowsleySt Helens and Wigan.
